The Medical Assistant is responsible for care of patients, including taking vital signs, performing laboratory procedures, performing in-house diagnostic testing and injections, and assisting providers with basic procedures.  In addition, s/he will specifically manage activities associated with the laboratory, hazardous waste disposal and supplies.  S/he is expected to provide this care in a sensitive and non-judgmental manner, to maintain a high degree of professional competence and the highest possible ethical standards, and to function as part of a care-giving team.

Curry Senior Center, a Tenderloin based non-profit geriatric health and social services agency serving low-income seniors, seeks an experienced Case Manager. An integral component of Curry Senior Center’s services, which include primary medical care, substance abuse treatment, mental health services, housing, community programs and congregate meals, the goal of case management is to facilitate the provision of necessary services to enable seniors to function independently in the community.

Position Summary:

The Mission SRO Collaborative is a partnership between MNRC, Dolores Street Community Services and St. Peter’s Housing Committee which works with SRO tenants to improve conditions of life in the SRO hotels of the Mission District.  The SRO Advocate will work primarily to conduct community organizing, educational activities, and social programs with SRO tenants.  The SRO Advocate will coordinate a team of SRO tenants to do outreach and activities in the hotels, including fire prevention and disaster preparedness workshops in coordination with relevant agencies and City departments.

As a valuable addition to our diverse and committed team of multi-disciplinary professionals, the clinical supervisor will carry a clinical caseload, provide program and clinical leadership, supervise 3 counseling FTEs, and ensure that program goals are met and high quality services are provided. Willingness to report for duty in an emergency disaster.

Position Summary:

The Comprehensive Clinical Case Manager is responsible for providing clinical case management services for an active caseload of clients who receive primary care at Lyon Martin Health Services.  The Case Manager will work with clients with complex psychosocial issues including those who are HIV positive and/or homeless, substance using, mentally ill, disabled, or otherwise in need of intervention.  Job functions will include intake, assessment, development of written collaborative care plans, care coordination, counseling, advocacy, referrals, follow-up, and documentation in client medical record.  The Case Manager will provide services on site at the clinic as well as broker services with and on behalf of the client while in the community.  The Case Manager will also work closely with Mental Health and Primary Care clinicians in coordinating care.
