SFCCC and OCHIN Collaborate for Patient Care Improvements through Epic Implementation
Through a grant provided by the Health Resources and Services Administration (HRSA), the San Francisco Community Clinic Consortium (SFCCC) has taken a leading role in improving patient care in San Francisco by migrating many of our member health centers onto a common Electronic Health Record (EHR) platform. Mission Neighborhood Health Center, HealthRIGHT360, St. Anthony Medical Clinic, South of Market Health Center, San Francisco Community Health Center, and SFCCC’s Street Outreach Services will all participate in applying the funding towards the EHR conversion.
The EHR, Epic, will be hosted and supported by OCHIN, a non-profit organization focused on supporting community health centers. Epic is also the EHR system used by many hospitals and other health care agencies in San Francisco and beyond. With organizations using the same software, patient care collaboration and back-end systems will be more streamlined, allowing primary care clinics to apply more of their resources directly to patient care services.
